HART of Folsom takes a grassroots, person-centered approach to reducing homelessness. Through partnerships with local churches, businesses, volunteers, and other nonprofits, we offer transitional housing, winter shelter, and targeted services to those experiencing homelessness. By offering consistent, relationship-based support, we help individuals not just find shelter—but stay housed long-term. Every person helped represents not just a statistic, but a step toward a more stable, compassionate city.

Each client is paired with a trained case manager who works one-on-one with them to identify the root causes of their housing instability. Through this trusted relationship, we guide individuals in setting realistic goals, tracking progress, and overcoming systemic and personal challenges. Our case managers don’t just offer advice—they offer accountability, advocacy, and care through every step of the journey.

We believe the goal of housing is more than just shelter—it’s stability. HART fosters long-term independence by equipping clients with the tools and confidence they need to move forward on their own. Through life skills training, financial guidance, and emotional support, we help individuals build sustainable lives where they can thrive, not just survive.

No one organization can meet every need. That’s why HART partners with a network of local, county, and state agencies to connect individuals to essential services—including healthcare, mental health counseling, legal aid, and housing assistance. By serving as a trusted hub, we ensure clients don’t fall through the cracks and can access the support necessary to move forward.

Financial instability is often both a cause and a result of homelessness. HART helps clients develop realistic budgets, understand their income and expenses, and set financial goals. With hands-on support, we demystify money management and give individuals the confidence to handle rent, bills, and savings. This foundational skill is key to preventing a return to homelessness.

Employment is a critical step toward independence. HART supports clients in finding and keeping jobs by helping them create resumes, prepare for interviews, secure proper identification, and connect with job training programs. Whether it’s helping someone rebuild their work history or navigate past legal barriers, we meet each individual where they are and walk with them toward stability.

HART actively seeks out rental units, transitional housing, and supportive housing opportunities within Folsom and surrounding areas. We work with landlords and housing providers willing to support individuals as they rebuild their lives. Through these partnerships, and with advocacy from our team, we open doors—literally and figuratively—for people ready to move forward.

HART of Folsom launched its transitional housing program to help individuals break the cycle of homelessness and start anew.

Clients work with a professional case manager to identify root causes of homelessness, set goals, and access life-changing resources.

The program currently operates short-term housing in two Folsom locations, with most clients graduating within six months to one year.

As housing insecurity rises, HART is expanding its Transitional Housing Program program and helping more people rebuild their lives.
Our Impact
In the last 18 months, 10 Folsom residents have transitioned out of homelessness into stable and permanent housing, and 7 more homeless residents are housed in our program today - for a total of 17 people who changed their circumstances at the remarkably low cost of less than $7,900 per person annually.
Folsom residents want us to expand our capacity, and with the building project at Oak Hills Church, we will increase the number of people we can help per year.
Our progress reflects the power of community support, compassion, consistency, and our clients' willingness to work hard. We're encouraged by the commitment of everyone who is a part of HART and remain committed to deep, sustainable change.

Currently, HART’s engineer is working with the Folsom Building Department to secure our building permit. Forever Tiny Homes, based in Cottonwood, CA., is the selected manufacturer for the new studio units on Oak Hills Church campus. We’re actively raising funds to purchase 5 of them and would love your support!

Winter Shelter
The Folsom Winter Shelter (FWS) is a seasonal, community-supported shelter that provides a warm, safe place to sleep, a hot meal, and compassionate connection for unhoused individuals in Folsom.
